Cost: $5 per quizzer plus meal cost.
Menu: Spaghetti, toss salad, fruit salad, rolls and a brownie for dessert; Cost is $5 per person. There is a Subway next door and a Safeway Deli across the street if you would like something else.

Please include the following information to me no later than Wednesday, January 28.
- Church Name
- Coach name, phone number and email
- Team Name(s), Level(s) desired (expert, intermediate or novice), 2nd choice of levels (if first choice wasn't expert)
- Names of all quizzers and what team they will be on. Please compress the number of your teams so one or two quizzers absent does not change the number of teams that will be at the tournament. (If changes to the number of teams and/or level of teams you are bringing occur after January 28 I need to know ASAP.)
- Names of any other helpers coming from your church, i.e. quiz masters, scorekeepers with email address or phone number. We need as many knowledgeable quiz masters and scorekeepers as possible.
- I need to know if you will not be there on Friday night or later than 6:30pm.
- Number of people eating Saturday lunch at the church.
